Director, Integrated Care

Reporting to the Executive Director, Health Systems Integration, the Director, Integrated Care is responsible for leading a team of people to deliver innovative projects, programs and services to facilitate, implement and scale integrated person-centred models of care across health and care settings. This includes primary, community and acute care, with a focus on reducing avoidable hospitalisations and improving shared care and communication across these care settings.

The Director will ensure a coordinated approach across all programs and teams, strategically advising on integrated care priorities and opportunities for alignment with other NWMPHN programs. The Director will also establish efficient processes to support ongoing and timely performance monitoring, driving a culture of improvement, innovation and accountability in the team.

The core responsibilities of the role will be in four distinct areas:

•   Ensuring the provision of commissioned services and programs are outcomes focused, efficient and effective, to drive accessible and integrated care and support to consumers, particularly those at risk of poor health outcomes.

•   Co-designing strategies with stakeholders to ensure that commissioned services and programs address priority needs and are linked to an evidence base.

•   Pro-actively lead and manage key stakeholders, including relevant governance and advisory groups, peak bodies, consumers and other relevant parties to build sustainable and meaningful relationships that inform strategy and drive improved outcomes.

•   Within a team environment drive a culture of collaboration, high performance and accountability that aligns capability to NWPHN’s priorities and builds a link between team performance and organizational outcomes.

To be considered for this complex but exciting vacancy, you will be tertiary qualified in a health-related discipline, ideally at a Post-graduate level. You will have proven skills and experience working in a health or government sector organisation and a sound understanding and appreciation of program management, ideally across a broad spectrum of health services. You’ll be a skilled communicator, authentic and purposeful leader, passionate and committed to evidence-based health reform. You’ll enjoy working as part of a team, but also be able to work with ambiguity and autonomy. Your background may include policy, health service or population health planning with a sound understanding of the primary/community health sector. If you’re a lateral thinker, outcome-focused and good at engaging with and influencing stakeholders then you should seriously consider expressing an interest in this position.
